Primary health care (PHC) is how individuals and families stay healthy. It is usually the first place you go for checkups, when you feel sick, or when you have health questions. Primary care providers (PCPs) get to know you over time and help guide your care as your needs change.
PHC focuses on preventing illness, not just treating it. Regular checkups, vaccines, screenings, and help managing conditions like asthma or diabetes can stop small problems from becoming serious. Many PCPs also support mental and emotional health.
Health is shaped by more than medical visits. Safe housing, healthy food, clean air, education, and community support all affect well-being. PHC helps connect families to community resources that support everyday health.
The National Alliance Ending Health Disparities (NAEHD) aligns closely with these goals. NAEHD works to ensure everyone has a fair chance to be healthy, no matter their background or where they live. It supports access to quality PHC and addresses the conditions that create health gaps.
Together, PHC and NAEHD promote fairness, prevention, and community-based care—helping individuals, families, and communities live healthier lives.
